A limited understanding of the "Business Problem" the project was started to address usually surfaces as the fundamental reason for project failure.

My definition of a failed project is one that does not meet the customers expectations by addressing the defined business problem and therefore adds little value to the business through it's (the project) implementation as a viable solution. Therefore, a project can be delivered and still be considered a failure.
